Policies and Fees

Appointment Policy
Appointments are available and can be made by calling 561-318-3727. An initial consultation will range from 1/2 to 1  hour, depending on the complexity of your case and number of conditions that need to be addressed.

New Patients
Dr. Porrata is currently accepting new patients.

Our Fees
There is no initial financial commitment or prepaid packages. Fees are due at the time of the appointment only. You will be charged only for the scheduled visits. Lifestyle Medicine Specialists accepts Visa, MasterCard,American Express, Care Credit, cash and checks.

Bariatric Medicine/Lifestyle Intervention:

Individual Visits (one patient):

1st visit: $175 this will include review of medical history, behavioral and nutrition assessment, request for appropriate diagnostic tests.*
2nd visit: $125 Physical exam,review of tests results, comprehensive nutrition, behavioral and excersise plan. Referal for specialist consults if indicated.*
Follow up visits: $125 review of medical, behavioral and nutrition progress.

Couples Visits:
1st visit: $300 this will include review of medical history, behavioral and nutrition assessment, request for appropriate diagnostic tests.*
2nd visit: $200 Physical exam,review of tests results, comprehensive nutrition, behavioral and excersise plan. Referal for specialist consults if indicated.*
Follow up visits: $200 review of medical, behavioral and nutrition progress.
We often see couples seeking treatment together.

* $100 will be added on increments of half hour past the hour visit, to both individual visits and couples visits.

Preventive Medicine Visits:
Vists are charged by time, at intervals of 15 mins.
Intervals: $100.00

Phone Consultation Policies:
Phone Consultations are availble on certain occasions at the same rate of an office visit. These consultations are taken by Dr. Porrata and scheduled during regular office hours and will be treated as such. After hour phone consultations will be answered by Dr. Porrata after regular office hours (8 am - 5pm). 

Monday- Friday: $50.00
Saturday- Sunday: $85.00

IV Therapy:

$150-$175

 per session. Initial consult needed, Follow up consults will be scheduled as needed.

Groups
We will arrange a first introductory visit to groups of three or more. The cost for the visit will be $100 per person. In this visit we will review the overall plan and the nutrition plan in general. Visits will have to continue as individual visits from then on as our plan is individually tailored and the Patient-Doctor relationship is confidential. Individual fees will apply.

Insurance Policy
The only item in our service not covered by insurance are the office visit fees. All Diagnostic tests and most medications are covered by your insurance carrier. We do not participate in any insurance programs. However, at your request, we can provide you with documentation that includes appropriate diagnosis codes and shows that you have paid in full. This documentation can be submitted to your insurance company. If your policy has out-of-network benefits you will be reimbursed accordingly. Dr. Porrata will provide you with a prescription for any blood work or testing so that you can take this to a lab or testing facility that is in your insurance network, same case with any other diagnostic testing needed or referral for specialist consults. If you are currently or have recently been seen by a primary care physician or specialist, we will request your prior records. This will allow us to avoid duplication of diagnostic tests. We will also provide you with all the documentation needed to submit for your insurance carrier or life insurance carrier to adjust their premiums once your risk factors and weight have improved. This type of intervention is also tax deductible and you should submit it for your CPA to review.

Cancellation Policy
Since Dr Porrata schedules her patients so there is minimal wait time and your appointment is not rushed, we exclusively reserve your appointment time to discuss all your concerns. In order to best serve all of our patients, we will accept cancellations if given 24 hours in advance and will gladly reschedule you for another time. Except for emergencies, if there is less than a 24 hour notice for a cancellation, a cancellation fee of 50% of the value of the scheduled visit will be charged.

Return Policy
There are no refunds on opened supplements. We will gladly offer an exchange or a refund on any unopened supplements.
